What is Gonorrhoea?

Gonorrhoea has been recognized for centuries, with descriptions appearing in ancient texts dating back to biblical times. The bacterium *Neisseria gonorrhoeae* was first identified in 1879. The introduction of antibiotics revolutionized its treatment, though antibiotic resistance has become a significant public health challenge in recent decades.

Symptoms

  • Many individuals with Gonorrhoea have no symptoms (asymptomatic).
  • For men: Painful urination, pus-like discharge from the penis, testicular pain or swelling.
  • For women: Increased vaginal discharge, painful urination, vaginal bleeding between periods, lower abdominal pain.
  • Other sites: Rectal infections (itching, soreness, bleeding, discharge), throat infections (sore throat), eye infections (pain, pus-like discharge).

Transmission

Gonorrhoea is transmitted through unprotected sexual contact, including vaginal, anal, and oral sex, with an infected person. It can also be passed from a pregnant person to their baby during childbirth, which can lead to severe eye infections in the newborn.

Contagious Period: Varies by disease

Prevention

  • Consistent and correct use of condoms during all sexual activity (vaginal, anal, oral).
  • Abstinence from sexual activity.
  • Regular STI testing, especially with new or multiple partners, and open communication with partners.
  • Prompt treatment of all sexual partners if diagnosed with Gonorrhoea to prevent reinfection and further spread.
  • No vaccine is currently available for Gonorrhoea.

There were 5866 diagnoses of syphilis and 44,210 gonorrhoea diagnoses in Australia in 2024 -- double what they were a decade ago. In 2015, Thailand began surveillance for N. gonorrhoeae urethritis as part of the World Health Organization Enhanced Gonococcal Antimicrobial Surveillance Programme (EGASP; https://www.who.int/initiatives/gonococcal-antimicrobial-surveillance-programme). Through EGASP surveillance, 31 urethritis-causing N. meningitidis isolates were collected from men in Thailand during 2017-2023 (10). All cases were successfully treated with 250 mg or 500 mg intramuscular ceftriaxone in accordance with national guidelines for male patients with urethral symptoms and gram-negative intracellular diplococci. The national reference laboratory (accredited according to ISO 15189:2012) confirmed N. meningitidis in isolates by using culture and biochemical characteristics of Neisseria spp. bacteria.
