El Salvador is a low-to-moderate-risk destination for health, with dengue fever being the most significant mosquito-borne threat, especially during and after the rainy season (May through October). Malaria risk is very low but not zero, primarily in rural lowland areas. Foodborne and waterborne illnesses are common risks, and tap water should not be consumed without treatment.

Malaria risk is very low and mostly limited to rural areas below 1,000 meters, particularly in Santa Ana, Ahuachapan, and La Union departments. Most travelers to cities and major tourist areas face negligible malaria risk.
Drink only bottled or purified water. Street food is widely available and popular but poses a risk of traveler's diarrhea. Stick to cooked foods from busy, reputable vendors and avoid raw salads in lower-quality establishments.
