What makes E. coli O157:H7 so dangerous?

O157:H7 produces Shiga toxin, which damages blood vessel linings, especially in kidneys and brain. About 5-10% of infections lead to hemolytic uremic syndrome (HUS)—a form of kidney failure. HUS is most common in children under 5 and can cause permanent kidney damage, brain damage, or death. This is why prompt diagnosis and avoiding anti-diarrheal medications are critical.

What is hemolytic uremic syndrome (HUS)?

HUS is a life-threatening complication where Shiga toxin destroys red blood cells and blocks kidney filtration. Symptoms: decreased urination, blood in urine, pale skin, extreme fatigue, irritability, small unexplained bruises. HUS requires hospitalization with blood transfusions and dialysis. Most children recover, but 12% have permanent kidney damage and 3-5% die.

Should I take anti-diarrheal medicine for E. coli?

NO. Anti-diarrheal medications (Imodium) may increase risk of HUS by slowing gut motility and allowing more toxin absorption. Antibiotics are also controversial—some studies suggest they may increase HUS risk by causing bacteria to release more toxin when dying. Seek medical evaluation rather than self-treating with medications.

What foods cause E. coli O157:H7?

Ground beef is the most common source (bacteria from cow intestines mixed during grinding). Also: raw milk, unpasteurized juice, raw vegetables (lettuce, sprouts), contaminated water, person-to-person in daycare/nursing homes. Whole cuts of beef are safer (bacteria only on surface, killed by cooking). Ground beef must reach 160°F throughout.

When should I see a doctor for E. coli?

Seek immediate medical care for: bloody diarrhea, severe stomach cramps, diarrhea lasting >3 days, fever >101.5°F, vomiting preventing fluid retention, signs of dehydration, or decreased urination (especially in children). Early recognition of HUS can be lifesaving. Children, elderly, and immunocompromised should seek care sooner.

How long does E. coli illness last?

Symptoms typically appear 3-4 days after exposure (range 1-10 days). Illness lasts 5-10 days in uncomplicated cases. However, bacteria can be shed in stool for weeks after recovery, requiring good hygiene to prevent spread. HUS typically develops 5-10 days after diarrhea starts.

Undercooked ground beef, contaminated produce, and unpasteurized dairy are common sources.
